Diffuser family does not actually attach to the Ceiling Grid?

Why is this? Lights attach and move to the ceiling grid, but diffusers dont?

Its the Revit OOTB family... I checked the family and its hosted on a generic extrusion, and don't think its ceiling hosted, is there a way to change that within the family?

Try these from the OOTB library:

These both should mount directly to ceilings.

Only a ceiling based family will follow the grid when it is moved and only when both the family and the ceiling are in the same model. A face-based family is only recognizing the face of the ceiling not the grid. There is no easy way (like a check box to tick) to make a face-based family a ceiling based family...other than remaking it.

I get around the grid movement issue by selecting the families that should move with the grid at the same time with the grid/ceiling. Pretty easy.
