I would like to create a family to show the span and direction of rafters. I attached a picture below. I've been trying to create one with some grips so I can drag it to the size I need. For the past few hours I've been trying to reverse engineer the sample structural beam tag and a different beam tag I found online with no luck. I am hoping there is an easy way to do this.. The only thing I can find about this subject is how to place a beam tag but not how to actually create one. What do most people do to get an annotation like this? Do people draw it manually each time?

In our office we currently achieve this with two line-based detail items and then a tag.
For wood structures we typically don't model each joist, but rather a floor with the thickness of the structural members included. So we tag the floor, rather than individual beams.
We show the span direction and extents with the attached detail items, but you could play around with the span direction symbol as well to achieve the desired effect.

Happy to help!
I think you could probably do this in one family, or as I mentioned with the span direction symbol. The latter might even be preferable, because it could automatically snap to the edges of your floor's extents.
Creating things at angles is a bit more tricky in Revit families, which is why I think I didn't do this in a single family when I originally created these. I'm sure it would be a worthwhile exercise to combine them though!
