In 2016 it is separate annotation. If it isn't there, just place a Stair Path annotation (Annotate Ribbon > Symbol panel).
I've also seen a graphic anomaly where the text appears and disappears. That's quirky as heck (graphics driver issue or bug/both) but the text prints even though it will be visible and then disappear as you zoom in/out or pan or stop.
As suggested, make sure UP is present in the annotation symbol family's settings.
Don't think so. Might as well just use text for that, nearly the same and more flexible (or a tag). I usually see that done added to a dimension string in enlarged stair plans fwiw.
