Stair: Runs: Extend Below Riser Base not working

yes this is one of those dumb revit things, it asks for an offset BELOW and so we assume that the value is already negative but the wording is poorly done in the parameter and its the same as an offset for anything else you need to enter the negative value as Elton suggested.
You're not alone here, I also scratched my head for a while trying to work out why my stair wasnt behaving a while back.

Are you kidding me? Well, no you are not 'cause I can see that you are right!
So how can it be that in some files you enter a positive number but in others you enter the reciprocal negative? I am happy to do it if it works, but that is odd.
