Current revision date is coming from the revision information on the sheet. You need to edit that information under the View>revisions, and then assign the correct cloud or revision on sheet to the sheet.

well yes because you added the prefix A.
the actual revision on the page is C (and you've added the prefix A) hence you see AC

        Remove the prefix.

Secondly, on your sheet (not in the revisions) you need to click on "revisions on the sheet" in the view properties when you activate the actual sheet you wish to modify. NOT THE TITLE BLOCK - just click in the view on blank space so that is the active page. It should say "VIEW PROPERTIES" not "A0 TITLEBLOCK" (for example)

Click on "revisions on the sheet" and you should see 3 revisions have been applied, hence it is arriving at revision C.

If this is not correct, remove the other 2 revisions and it will change to revision A.

The other problem may be that you are applying revisions "per project" (which is my guess - actually I can see you have this activated in your screenshot above)
In your revisions list, you need to make sure you check "per sheet" instead of "per project"

If you have it set to "per project" it applies automatically every new revision to every sheet in the project. If you wish to manage each sheet separately, you need to click "per sheet" and then apply the revisions individually as I explained in the process above.

If you wish to use revision clouds associated to the revision number, then you need to also select in the drop down menu "revision cloud" and when you draw a revision cloud on that particular sheet you must associate the correct revision number to the revision cloud, and it will automatically apply the correct revision as long as the revision clouds are visible on that sheet. If you do not wish to use clouds, then you need to check "none" in this drop down menu (Under the "show" column)

Per sheet vs per project is a company standards (and probably regional) decision. Neither way is right or wrong, it just depends on what you want to see on your sheets.
