Does anyone know if it is possible to include a type image in a tag? Using Revit 2017 - either OTB or via an addin?

Nope. Nice idea but I just tried it and unfortunately it doesnt work.
Image is not available in Labels. You can add an Image parameter as a shared parameter, but this only shows the name of the image as text.
What does work is if you put an image in the tag family, but then it is only one fixed image per tag not parametric.
