Ceiling Mounted Device - Mirrored on Y axis in RCP view?

So I have been using Revit off and on for several years. I have noticed on some family types where the device (security camera) is hosted to the ceiling plane and the annotative symbol shows correctly in Plan View, however it is mirrored on the Y axis (or rotated 90 degrees counter-clockwise) on the Reflected Ceiling View. I've never needed a solution until now. I've done very minimal family modifications in the past, but this one completely stumps me. At this point, I am thinking I need a new family built. Anyone have any experience with this type of problem?

When you open the family what is the relationship between the symbol and the work planes that "Defines Origin". The center of the symbol should be centered over those work planes.

The 2 reference plans are indeed assigned as "Defines Origin". It looks like "Maintain Annotation Orientation" has something to do with my issue. When unselected the annotation is the same for both Plan and Reflected Views.
