    CSV file column header format...

Do I need to be mindful of header format in CSV lookup files?

I know of the "##" separators but are spaces allowed? Underscores? Etc?

    Yes.

The "##" are required. Spaces and underscores in the name of the parameter in the lookup table are permitted.

However, the name of the parameter in the headers of a lookup table are usually abbreviations, such as "FOD". The name of these parameters do not have to be descriptive because the .csv file is usually edited by the family maker, only, not by the final user. In the Family Types window, you do need to use the descriptive name, "Fitting Outside Parameter", which gets its value from the "FOD" parameter in the lookup table, which is written as FOD##length##inches or FOD##length##millimeters
