I thought you could copy and paste a level within the family but that does not work. There is also no tool for adding one.
This is for a family and not a family template. If you could give me an answer for both types that would be great. Thank you.
As you have found this is not possible. You could rebuild your family starting with a two level template or nest your existing family into a new two level family.

As Elton said, not possible. Why not just add a reference plane, name it Level 2 and create a parameter for level to level dimension and get on with life?

For things like elevators, that gets to be a chore to do the math for how tall it is.
But, as has already been said: either rebuild in a 2 level family template, or nest it in to a 2 level family template.
